A Consideration on The Operative Methods for The Peritonitis Caused by Ileum Perforation

Abstract
Peritonitis caused by the perforztion of the ileum often is a complications of the systemic diseases such as typhoid fever cr other kinds of salmonellosis. Since this complication usuallc takes place at the second or third week of the disease course, the patient¢¥s condition is generally poor. .The choice of the methods or surgical operation for the peritonitis depends upon the general condition of the patient and upon the intraabdeminal state.
During 5 years (1957-1962), of the 141 cases, 80 cases were operated by the method of the debridement and simple closure, 60 by resection of the involved intestine followed by end-to-endanastomosis and one case was treated by exteriorization of the involved ileum expecting the secondary surgical intervention after his general conditic;n had improved.
The rates of the mortality and of the reperforation were higher in the former group.
Since Oct. 1962, we treated 46 cases of the peritonitis by the method of simple closure and resection alternately except extremely poor risk patients in attempt to establish more favorable result.
The result cf these studies shows that the mortalit~r rate was lower in the resection group and postoperative complications were almost epual.
